📣 2025–2026 Physical and Forms Information

Attention all swimmers and water polo athletes:

✅ Incoming Freshmen and Rising Juniors
A new physical is required for participation.

• Information about the NISD-offered physical has been sent home with student-athletes.
• Completed physical forms must be turned in directly to the athletic trainers or the middle school head coach.

✅ Rising Sophomores and Rising Seniors
An updated Medical History Form is required.

• Forms must be turned in directly to the athletic trainers.

✅ All Athletes
All athletes must complete the required RankOne online forms before participating in practices or competitions.

📄 Important:
Failure to complete and submit all required documents will delay an athlete’s clearance for participation.
